The Champion Leadership Academy

Champion Gym hereby grants, and the undersigned member accepts, membership upon payment of the first and last monthly membership dues, defaulting afterward monthly, entitling the member to the use of Champion Gym facilities without obligation to any financial contract. This also applies to time block memberships of any kind, such as 6 or 12 months, as well as Day Pass memberships. All monthly membership payments must have a valid credit/debit or bank account method of auto-withdrawal on file to access the monthly rate.  All memberships, regardless of configuration, are subject to the rules and regulations, including the Champion Code of Conduct below. For Champion Adventure Academy or Adventure Camps, including PD days, I agree that my purchase is nonrefundable once the camp is fully booked, in use, or after 7 days prior to the first camp day, whichever comes first (except in cases of provable medical circumstances). Champion Gym reserves the right to cancel said membership for violation of the terms and conditions presented here and/or the code of conduct. Champion Gym may agree to postpone camp weeks, upon submission of appropriate documentation and satisfaction of certain conditions. Camps and Academies are transferable in whole, provided sign-up requirements such as age are met. For general memberships, after the initial term, a member may terminate membership in writing at least 30 days prior to their next scheduled payment. Writing can include email with an enclosed signature. Champion Gym reserves the right to cancel said membership for violation of the same rules, regulations, and/or code of conduct and/or as the owner sees fit. Champion Gym does not transfer joining fees or monthly dues outside of the immediate family unit. Champion Gym may agree to temporarily freeze monthly payments, provided advance notice is given, for a minimum of 2 weeks up to a maximum of 1 month in total within each calendar year. If there is no notice or the freeze exceeds 1 month, auto-withdrawal will re-continue. If auto-withdrawal fails for any reason, CHAMPION GYM reserves the right to cancel said membership, and the client forfeits their last month. There are no refunds for forgetting to cancel, verbal cancellations (in person or on the phone), or recurring auto-withdrawals after a client's scheduled freeze resumes.

I understand I and/or my children may be photographed or filmed while attending CHAMPION GYM training facility or their activities outside of the facility, and I give permission to CHAMPION GYM & any affiliates to use any & all photos, video footage, and/or video streaming for promotional, sales, publicity, & advertising purposes on all media, including the internet. I understand that under no circumstances shall the information, techniques, or other skills learned at the CHAMPION GYM training facility or by the trainers be used outside the school unless required for legitimate sport or self-defence. I confirm that there will be no binding verbal presentations other than those specified in this agreement. Failure to use the facility or camp in whole or in part will not relieve members of payment. I understand memberships are non-transferable. For personal training, 24 hours' notice is required for cancellation, or as soon as possible in cases of an emergency. No-shows and cancellations without 24 hours' notice are subject to the full session fee, except in the case of an actual, verifiable emergency. If you are late, you will have less time with your trainer and will still be billed for the entire session. On behalf of myself, members of my family, my heirs, executors, administrators & assigns, I hereby forever release, discharge & hold harmless 1544650 AB Ltd. o/a CHAMPION GYM, representatives & agents for any illness, injury, loss, or damage to my person, that of my child or property howsoever caused, arising out of or in connection with me or my child taking part in the practice or competition of Martial Arts and Sports including MMA, Kickboxing, Brazilian Jiu-Jitsu, Wrestling, Ninja Warrior, Grappling, Krav Maga, Gymnastics, EXCITE NIGHT including BattleChampz, Go-Karts, Lazer Tag & any variation of activities including classes, as well as any associated activities including any Pro-D Day and Adventure Camps in or outdoors, circuit training & notwithstanding that the same may have been contributed to or occasioned by the negligence of CHAMPION GYM representatives or agents & associated leaseholder, and the City of Grande Prairie whereupon CHAMPION GYM activities may be conducted.

I have read all the terms & conditions of this agreement & acknowledge full responsibility to those stated herein as required for registration with any Champion Gym membership.
